Clip1:

Through the left nasal cavity, sphenoid ostium is identified, opened, and widened.

 

 

 

Clip2:

The sphenoid sinus is entered into and the opening is widened; the sinus is cleared off the mucosa.

 

 

 

Clip3:

The floor of the pit.fossa is exposed, opened with a chisel, and the opening is widened.

 
   

Clip4:

The dura is cauterized, opened in a cruciate fashion. The tumor is excised with punch, curette, and suction.

Following excision, the diaphgrama sella is visualized. Further clearance of the tumor is carried out. The sella is packed with fat at the end of the procedure to prevent CSF rhinorrhea.
